Please read these Terms of Use carefully before using the Cardiograma platform. Using the platform means you fully accept these terms.
1. Service identification and responsible entity
The Cardiograma platform is operated by Infratartism, Lda., Portuguese legal entity with tax number (NIF) 517284707, owner of the Cardiograma domain, brand, and application.
Cardiograma is a digital cardiovascular follow-up platform that provides online and in-person medical consultations, health data monitoring, and continuous clinical support by a specialist cardiologist.
The platform is managed under the clinical supervision of Dr. Mafalda Carrington, Clinical Director and Cardiologist, registered with the Portuguese Medical Association under number OM60673.
In-person consultations are provided at Clínica Jardim Botânico, Porto, registered with ERS under number 180108.
2. Acceptance of terms
By accessing or using Cardiograma, you fully accept these Terms of Use. If you disagree with any part, you must stop using the platform.
These terms may be updated from time to time. Changes become effective upon publication on this page with an updated date. Continued use after changes means acceptance.
3. Nature of medical service
Cardiograma provides a regulated digital healthcare service. Consultations on the platform are medical acts performed by a physician legally licensed to practice medicine in Portugal.
The service does not replace emergency care. In a medical emergency, users must immediately call 112 or go to the nearest emergency department.
The cardiovascular follow-up program complements national healthcare services and should not be considered a replacement for your family doctor or other specialists.
All medical acts are subject to medical confidentiality and professional secrecy under applicable law and the Portuguese Medical Association Code of Ethics.
4. Eligibility and registration
To use the platform, the user must:
- Be at least 18 years old. Cases involving minors are only accepted after legal/clinical assessment and legal representative involvement;
- Provide truthful, complete, and up-to-date information during registration;
- Keep account credentials confidential;
- Immediately notify Cardiograma of unauthorized account use.
Infratartism, Lda. reserves the right to refuse or terminate accounts in case of breach of these terms or false information.
5. User obligations
The user agrees to:
- Provide complete and truthful clinical information, including history, medication, and allergies;
- Not use the platform for unlawful or unethical purposes;
- Not share account access with third parties;
- Respect healthcare professionals and support staff;
- Attend scheduled consultations or cancel with at least 24 hours notice;
- Not reproduce, distribute, or disclose clinical content/records without explicit authorization.
6. Connected health devices, accounts, and in-app logs
Users may optionally connect health devices/accounts. Currently supported connections are Apple Health (HealthKit) in the iOS app and Health Connect in the Android app (data from any devices/apps connected to these platforms on the phone).
By connecting these sources, users authorize Cardiograma to read only the data needed for clinical follow-up. These integrations are also subject to the terms and policies of each platform/manufacturer.
Users are responsible for the accuracy of device-generated data and may disconnect integrations at any time. Processing is governed by the Cardiograma Privacy Policy.
Users may also voluntarily record in the app manual measurements (e.g., blood pressure and cholesterol), meals (Nutrition section), and smoking-cessation follow-up data (Smoking section). These logs support clinical follow-up and may be viewed by the physicians who follow the user on the platform. AI estimates and suggestions in the Nutrition section are optional, informational, and do not replace medical advice.
7. Scheduling and cancellation
Online consultations are scheduled through the platform and are confirmed after booking completion and payment validation when applicable.
Standard cancellation/refund rule for paid online consultations:
- Cancellation at least 24 hours before: eligible for refund according to the policy active at booking time;
- Cancellation under 24 hours: generally non-refundable, except in duly justified clinical/commercial exceptions.
Specific refund rules may depend on payment method and scheduling integration and are always shown during booking/checkout.
For in-person consultations at Clínica Jardim Botânico (Porto), booking/cancellation is also subject to that clinic's policy.
8. Pricing and payments
Consultation and program prices are communicated before any financial commitment is confirmed.
The program may include a portion of fees linked to adherence to agreed follow-up objectives. This does not constitute any guarantee of clinical outcomes or cure, which cannot be guaranteed in medical practice.
Payments are processed through secure and certified payment providers.
9. Intellectual property
All platform content, including text, logos, design, software, and clinical materials, is owned by Infratartism, Lda. or its licensors and protected by copyright and other intellectual property laws.
Reproduction, distribution, modification, or commercial use without prior written authorization is prohibited.
10. Limitation of liability
Cardiograma is not liable for:
- Medical decisions taken by users based on information obtained outside platform consultations;
- Temporary service interruptions for technical/maintenance reasons;
- Damages resulting from improper platform use or incorrect information provided by users;
- Failures in third-party monitoring devices connected to the platform.
Under no circumstances shall Infratartism, Lda.'s total liability exceed the amount paid for services in the previous 12 months.
11. Termination
Users may close their account at any time through profile settings or direct contact.
Infratartism, Lda. may suspend or terminate access without prior notice in case of breach of these terms, abusive behavior, or clinical safety reasons.
Account closure ends platform access but does not affect rights/obligations from previously contracted consultations/programs.
12. Governing law and jurisdiction
These Terms of Use are governed by Portuguese law and the applicable legal framework for healthcare, consumer protection, and data protection.
Without prejudice to mandatory consumer protections and legally applicable ADR mechanisms, disputes arising from platform use are subject to the jurisdiction of the competent Portuguese courts.
13. Lei aplicável e foro
Estes Termos de Uso regem-se pela lei portuguesa e pelo quadro legal aplicável em matéria de saúde, consumo e proteção de dados.
Sem prejuízo de normas imperativas de proteção do consumidor e de meios alternativos de resolução de litígios legalmente previstos, as partes submetem os litígios emergentes da utilização da plataforma à jurisdição dos tribunais portugueses competentes.
